Each license entitles one pharmacy to use the system for as long as that pharmacy exists. To use the SentinelSM system in more than one pharmacy, you must purchase additional licenses. In agreeing to the terms of this sale you agree with all statements and conditions set forth here. You also will have the right to use the Quality ManagerSM on line reporting and analysis system. There will be an annual fee to continue using the Quality ManagerSM program and your data contained therein. HIPAA does not apply to the Quality ManagerSM program as it does not ask for or contain any patient identifiable information. You may not enter any patient identifiable information into the Quality ManagerSM data collection site. The SentinelSM system and the Quality ManagerSM program and all material contained in the Guidelines and Tools are copyrighted materials, trademarks and/or service marks of National Alliance of State Pharmacy Associations, LLC You further understand and agree that:
