Pharmacy Quality Commitment^®, Quality Manager^SM on line error reporting and analysis program and Sentinel^SM system are trademarks and/or service marks of Pharmacy Quality Commitment®

You are purchasing a site license to use the Sentinel^SM system in your pharmacy or pharmacies. Each license entitles one pharmacy to use the system for as long as that pharmacy exists. To use the Sentinel^SM system in more than one pharmacy, you must purchase additional licenses. In agreeing to the terms of this sale you agree with all statements and conditions set forth here. You also will have the right to use the Quality Manager^SM on line reporting and analysis system.  There will be an annual fee to continue using the Quality Manager^SM program and your data contained therein. HIPAA does not apply to the Quality Manager^SM program as it does not ask for or contain any patient identifiable information. You may not enter any patient identifiable information into the Quality Manager^SM data collection site.

• No warranties are made, expressed or implied. Any implied warranties are null and void to the extent allowed by law.
   
• The Sentinel^SM system is a risk management service that is designed to assist in the reduction of pharmacy errors in community pharmacies. The Quality Manager^SM audit system is a self reporting system of tracking mistakes, near-misses and errors. The Sentinel^SM System and the Quality Manager^SM audit system are only effective to the extent they are used. Neither the Quality Manager^SM audit system nor the Sentinel^SM system can prevent all errors and they do not promise no errors. No system can replace the care and diligence of the pharmacist and pharmacy technician. The Sentinel^SM system gives a method to build quality habits using best pharmacy practices in a regular workflow.
   
• The Guidelines are not unbendable, unchangeable policies or procedures. As with any quality system, they should be monitored and modified to fit a particular environment.
   
• ASSIGNMENT. NASPA shall have the right to assign this Agreement to any person, firm or corporation, without prior consent.
